Key Responsibilities\n

• Conduct comprehensive assessments to identify and address the social and emotional needs of patients and their families, promoting a supportive atmosphere for recovery.

• Actively collaborate with an interdisciplinary team to develop, implement, and refine discharge plans that facilitate smooth transitions from care back to daily life.

• Maintain timely and accurate documentation of patient interactions, progress notes, and treatment plans within our electronic health records system, ensuring compliance and quality of care.

Qualifications\n
