Key Responsibilities\n

• Conduct thorough initial evaluations of patients' emotional and mental health needs, translating those assessments into tailored treatment plans that reflect their unique circumstances.

• Support patients in accomplishing their personal goals by providing pertinent resources, emotional support, and guidance throughout their hospice journey.

• Regularly monitor patient progress, documenting changes and modifying treatment plans as necessary to ensure each patient's needs are met throughout their care.

• Actively coordinate with healthcare providers, social service agencies, and community organizations to facilitate access to various services beneficial for patients and their families.

• Stay informed on the latest practices and advancements in mental health support through continuing education, ensuring you remain compliant with licensure requirements.

• Offer initial assessments and regular check-ins, providing referrals to specialized services as needed to enhance patient care.

Patient Population / Client Focus\n

The primary focus of this role is on patients receiving hospice care, many of whom face complex emotional and psychological challenges related to terminal illnesses. You will work to build trust and provide compassionate support to those who are navigating the end-of-life process, as well as their families, by fostering an environment of understanding and respect.

Qualifications\n

• Must hold a Master’s degree in Social Work, Counseling, Psychology, or another related field that addresses mental health.

• Possess a current and independently issued Licensed Clinical Social Worker (LCSW) license.

• Be a resident of California, maintaining knowledge of state regulations and requirements relevant to hospice and mental health services.

• Demonstrate high proficiency in technology, particularly with EHR systems, to streamline patient documentation and case management.

• Exhibit excellent verbal and written communication skills, ensuring timely and precise documentation of patient interactions and treatment plans.

Preferred Experience\n

Compensation and Benefits\n

This position offers a competitive salary ranging from $80,000 to $122,000 per year, commensurate with experience and qualifications.
