Work Environment

This position is fully onsite, providing therapy in various home settings. Our physical therapists engage with patients in their residences, which can vary from private homes to assisted living facilities. Key Responsibilities

• Adhere to state practice acts and maintain compliance with regulations governing physical therapy practice and supervision.
• Conduct thorough assessments to develop individualized treatment plans for patients facing functional limitations or disabilities, using a hands-on approach that emphasizes patient participation.
• Perform evaluations that include comprehensive reviews of medical history, specific assessments, and generation of recommendations with detailed documentation of results.
• Demonstrate expertise in implementing treatment protocols based upon evaluation findings, effectively adjusting strategies as patients progress or as needs evolve.
• Consult with other healthcare providers as necessary, and modify care plans based on patient progress, emphasizing multidisciplinary collaboration.
• Prioritize efficient discharge planning and follow-up care, documenting the entire course of care including evaluations, treatment protocols, and progress notes to ensure continuity.
• Engage actively in ongoing training and education to stay current with industry standards and best practices.
• Participate in team meetings and initiatives aimed at improving access to care and patient outcomes.
• Document patient treatment effectively, including any changes in condition that may warrant a reassessment or adjustment in treatment strategy.

Patient Population / Client Focus

The ideal candidate will work primarily with orthopedic patients who require rehabilitation and recovery support. This includes individuals recovering from surgery, those undergoing rehabilitation post-injury, and senior patients managing chronic pain or mobility issues. Understanding the unique challenges faced by each patient in a home care setting is crucial to developing effective treatment strategies that empower patients towards independence and improved quality of life.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program.
• Valid state licensure as a Physical Therapist with the Physical Therapy Examining Committee.
• A minimum of two years of outpatient orthopedic experience is required, showcasing a solid understanding of treatment modalities applicable in various scenarios.
• Current CPR certification to ensure readiness for any medical emergencies.

Preferred Experience

• Experience in home health environments or with geriatric populations highly desirable.

Compensation and Benefits
