Work Environment

This role will require the successful candidate to deliver personalized therapy services at patients' homes, ensuring their comfort while receiving care. The position entails a 40-hour workweek, Monday through Friday, providing a structured yet dynamic work schedule as you meet patients in various environments. Our focus on home care allows us to be directly involved in the healing process in a familiar setting, catering to individual needs effectively.

Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians to develop customized treatment plans tailored to individual conditions and rehabilitation goals.
• Implement a variety of therapeutic techniques and interventions, including manual therapy, exercise prescription, and neuromuscular re-education, while continuously monitoring patient progress.
• Collaborate closely with a multidisciplinary team, sharing insights and implementing evidence-based strategies to maximize patient outcomes.
• Educate and empower patients and their caregivers on effective rehabilitation strategies, ensuring they understand their roles in the healing process.
• Maintain accurate and timely records of patient evaluations, treatment plans, and progress notes in compliance with healthcare regulations and organizational standards.
• Demonstrate exceptional customer service, fostering a supportive and motivating environment that contributes to patient satisfaction and engagement.
• Meet established utilization goals and actively participate in achieving departmental business objectives, contributing to the overall success of the organization.

Patient Population / Client Focus

The patients you will treat come from diverse backgrounds and range across various specialties, including orthopedic, pediatric, neurologic, and sports medicine cases. Your role is crucial in delivering rehabilitation services that help patients recover mobility and restore their daily functioning, ultimately impacting their overall quality of life.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid State License in Physical Therapy, ensuring you meet state regulatory standards.
• A minimum of two years of relevant experience in physical therapy, ideally with a focus on orthopedic cases.
• Current Basic Life Support Certification from the American Heart Association.
• Strong written and verbal communication skills to effectively interact with patients, families, and healthcare professionals.
• Proficiency in word processing and basic computer applications to maintain documentation and correspondence.
• A commitment to establishing strong patient relationships that encourage goal achievement and enhance patient engagement.
