Work Environment

This role is conducted onsite in patients' homes across Henderson and the surrounding areas. Working in this environment, you will have the opportunity to build meaningful relationships with patients and their families while creating a warm, supportive atmosphere for healing. Our home care setting allows for flexible working hours, typically aligned from Monday to Friday, 9 am to 5 pm, making it a convenient option for both our team and the families we serve.

Key Responsibilities

• Conduct comprehensive evaluations to assess patients' physical conditions and functional abilities using standardized assessments and personal interviews.
• Develop individualized treatment plans that address each patient's specific rehabilitation needs and long-term goals, regularly updating plans based on patient progress.
• Implement hands-on techniques, personalized therapeutic exercises, and evidence-based modalities to alleviate pain, enhance mobility, and facilitate functional recovery.
• Educate patients and their families on appropriate body mechanics, effective home exercise programs, and injury prevention strategies to promote wellness and self-management.
• Monitor patients' progress closely throughout the rehabilitation process, adjusting treatment protocols as necessary to optimize outcomes.
• Collaborate with other healthcare professionals, such as physicians and occupational therapists, to ensure a comprehensive approach to patient care.
• Maintain meticulous documentation of patient interactions, treatment interventions, and progress in compliance with organizational policies and legal standards.

Patient Population / Client Focus

Our clients include individuals of all ages who require physical rehabilitation due to various conditions, including but not limited to post-surgical recovery, sports injuries, chronic pain management, and mobility challenges related to age or physical disabilities. You will work closely with patients suffering from back and neck pain, osteoarthritis, tendonitis, sprains, fractures, and gait deficits. Each treatment plan is tailored to the unique needs of the patient, aimed at enhancing their overall quality of life and independence.

Qualifications

• A Bachelor's or Master's degree in Physical Therapy from an accredited institution.
• Current state licensure as a Physical Therapist, ensuring compliance with local regulations and standards of practice.
• Board certification from the American Physical Therapy Association (APTA) is preferred.
• A minimum of one year of clinical experience in physical therapy, demonstrating proficiency in assessment and treatment methodologies.
• Knowledge of and skills in utilizing a variety of therapeutic modalities, including electrical stimulation, ultrasound, and manual therapy techniques.
• Outstanding analytical and problem-solving abilities to effectively assess and address diverse patient rehabilitation needs.
• Excellent interpersonal and communication skills to foster trust and rapport with patients and their families.

Preferred Experience

While a minimum of one year of experience is required, candidates with additional experience in home health settings or specialty areas such as geriatrics, sports rehabilitation, or post-operative recovery will be highly regarded. Familiarity with electronic health record (EHR) systems is an advantage, facilitating efficient patient management and documentation.

Compensation and Benefits
