Key Responsibilities\n

• Collaborate closely with healthcare professionals, including physicians, nurses, and case managers, to develop comprehensive treatment plans tailored to the psychosocial needs of patients.

• Conduct thorough psychosocial assessments and develop targeted interventions to address the complex emotional and behavioral challenges faced by patients in hospice care.
\n
• Maintain up-to-date and accurate clinical documentation, including progress notes and treatment records, to comply with department protocols and enhance interdisciplinary communication.

• Provide empathetic counseling and crisis intervention services, supporting patients emotionally during transitions and evaluating their needs in times of crisis.

• Facilitate family consultations to help caregivers and support systems better understand the treatment process and participate actively in discharge planning.

Patient Population / Client Focus\n

This role focuses primarily on patients receiving palliative and hospice care, often grappling with advanced stages of illness. Your interventions will support both patients and their families, addressing end-of-life issues, emotional distress, grief, and the myriad challenges that come with terminal diagnoses. You will play a critical role in empowering families by providing them with resources, coping strategies, and emotional support during this vulnerable time.

Qualifications\n

• A Bachelor’s Degree in Social Work from an accredited institution is required.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.

• A minimum of 2 years of direct experience in a clinical environment, preferably within hospice or palliative care.

• Strong proficiency in computer skills, including familiarity with electronic medical records systems, word processing, and spreadsheet programs.

• Ability to efficiently prioritize and manage multiple tasks in a fast-paced and dynamic work environment.

Compensation and Benefits\n

The annual salary for this role ranges from $100,000 to $105,000, commensurate with experience, accompanied by a comprehensive benefits package. This package includes:
