Key Responsibilities\n

• Collaborate closely with a multidisciplinary healthcare team to develop comprehensive treatment plans tailored to individual patient needs, facilitating interventions grounded in psychosocial assessments.

• Conduct thorough assessments to identify patients' emotional and behavioral challenges; this includes timely documentation of clinical progress notes and treatment plans that adhere to department policies and standards.

• Provide counseling services and crisis intervention to clients facing acute emotional distress, ensuring immediate support and appropriate therapeutic interventions.

Patient Population / Client Focus\n

In this role, you will primarily serve a diverse population of clients grappling with serious illnesses, behavioral health challenges, and the complexities associated with hospice care. Your ability to relate to individuals from varying backgrounds and with different life experiences is vital, as is your skill in providing age-appropriate treatment across the lifespan. Qualifications\n

• A minimum of 3 years of unsupervised post-licensure experience in a clinical social work setting.
