This position offers a flexible and supportive work environment with an emphasis on remote work to help you best meet the clients' needs. Your primary duties will occur through virtual consultations and visits. The hospice center environment fosters collaboration between interdisciplinary teams, including healthcare professionals, caregivers, and volunteers. You will also have the opportunity to participate in team meetings and training sessions aimed at enhancing service delivery and personal growth.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate the emotional, mental, and social needs of clients, ensuring a thorough understanding of their situations.

• Develop individualized care plans in conjunction with clients and their families, aligning with their personal goals and preferences while addressing complex end-of-life issues.

• Provide ongoing counseling and emotional support to seniors and their families, helping them navigate difficult transitions and life changes.

• Advocate for clients' rights and access to necessary community resources, ensuring necessary services are readily available.

• Engage with interdisciplinary teams—comprising nurses, doctors, and other healthcare professionals—to deliver cohesive and comprehensive care.

• Regularly monitor and evaluate client progress through virtual check-ins and communication, documenting changes in their emotional and physical well-being.

• Facilitate discussions among family members dealing with care decisions, ensuring effective communication and conflict resolution.

• Educate clients and families about available resources in the community, including support groups, financial aid, and service programs to enrich their lives.

• Maintain meticulous records of client interactions, assessments, care plans, and communications in compliance with ethical and regulatory standards.

• Stay updated on best practices, regulations, and standards in social work to deliver the best care possible to clients.

Patient Population / Client Focus\n

Your primary focus will be on seniors within the hospice care spectrum, facing end-of-life challenges. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• A valid and active LMSW license issued by the state of New Jersey.

• Minimum of two years of relevant experience as a social worker, preferably in hospice, geriatric care, or a community-centered environment.

Compensation and Benefits\n
