Key Responsibilities\n

• Conduct detailed psychosocial assessments by engaging with patients through interviews, careful observation, and thorough review of medical histories to identify emotional, social, and environmental factors impacting recovery.

• Provide individualized counseling and emotional support to patients and their families, addressing adjustment challenges, grief reactions, and coping strategies.

• Collaborate closely with a multidisciplinary rehabilitation team to develop comprehensive treatment plans that address the specific psychosocial needs of each patient.

• Guide patients in accessing community resources such as support groups, vocational rehabilitation services, and housing opportunities to aid their recovery journey.

• Educate patients and their families on available community resources and post-rehabilitation services to ensure continuity of care.

• Actively participate in discharge planning processes, helping to facilitate smooth transitions to home or other care environments.

Patient Population / Client Focus\n

Our patient population includes individuals recovering from a wide range of conditions, including but not limited to stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n

Compensation and Benefits\n
