Key Responsibilities

• Conduct thorough assessments to determine the medical necessity of PT services and evaluate the appropriateness of treatment modalities based on nationally recognized clinical guidelines.
• Create and manage individualized Plans of Care that specify the length, intensity, and frequency of treatment, consistently updating them based on patient progress.
• Document all clinical findings, treatment progress, and educational opportunities in comprehensive reports that inform the multidisciplinary team and improve service delivery.
• Participate in peer consultations, providing expertise when treatment plans diverge from established guidelines, ensuring collaborative decision-making prior to submission to the Medical Director.
• Review clinical requests for PT services submitted by coordinators, ensuring all supporting documentation—diagnosis, evaluation, and progress notes—are thorough and timely.

Patient Population / Client Focus

Your role will involve working with a diverse patient demographic, including individuals recovering from physical injuries, those managing chronic illnesses, and pediatric patients requiring specialized attention. Your knowledge and skills will directly influence their rehabilitation journeys, helping them overcome barriers to recovery and achieve their personal health goals.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Active State Licensure as a Physical Therapist.
• Strong foundational knowledge of anatomy and physiology.
• Proficient in trigger point therapy techniques.
• Comfortable working with pediatric patients and in both clinic and hospital settings.
• Ability to navigate outpatient and acute care environments effectively.

Preferred Experience

• Experience in evaluating and treating a variety of physical and developmental conditions.
• Familiarity with hospital medicine practices and inpatient care protocols.
• History of collaborative work within a multidisciplinary team setting, promoting integrated care for patients.
