This role is structured within our Community Health Clinic model, where our clinicians work together in a supportive, collaborative environment. While physical interactions are vital, this position allows you the flexibility to provide services remotely, catering to the needs of families who may have difficulty accessing traditional in-person support. Key Responsibilities\n

• Conduct thorough evaluations and assessments of children to identify developmental delays and formulate Individualized Family Service Plans (IFSP).
\n
• Develop and implement these plans in collaboration with families and multidisciplinary team members, ensuring alignment with evidence-based practices.
\n
• Regularly monitor and evaluate the outcomes of interventions, adapting strategies as necessary to meet each child's evolving needs.
\n
• Provide educational resources, support, and strategies to families to enhance their children's developmental skills in a natural learning environment.
\n
• Engage families through effective communication and relationship-building, fostering a supportive atmosphere for discussions around developmental concerns.
\n
• Adhere to all local, state, and federal guidelines, ensuring compliance with health standards and procedural expectations.
\n
• Utilize the coaching model and natural environment teaching to facilitate learning and development opportunities.
\n
• Participate in team meetings and professional development sessions to continuously improve professional skills and expand knowledge of family systems and child development practices.

Patient Population / Client Focus\n

This role focuses predominantly on children, particularly those with disabilities or developmental delays, from birth to three years old. Your work will directly impact their early cognitive, social, emotional, and communication skill development. By partnering with families, you will provide vital assistance tailored to each child’s unique circumstances, creating strategies that contribute to positive developmental trajectories.

Qualifications\n

• Master's Degree in Social Work from an accredited institution or a Doctorate in Social Work.

• Current licensure as a Clinical Social Worker (LCSW) from the Department of Public Health in New York.
\n
• Valid driver’s license to enable travel for assessments or meetings, as needed.
\n
• Strong organizational skills with the ability to manage multiple cases and maintain accurate documentation of services delivered.

• Exceptional communication and interpersonal skills with a focus on building rapport and trust.

• A commitment to promoting equitable access to resources and support within diverse communities.

Preferred Experience\n

• A minimum of one year of experience working with young children in early intervention settings.

• Familiarity with Natural Learning Environment Practices and a commitment to using them in your approach.

Compensation and Benefits\n
