Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observations, and the review of medical records to formulate a comprehensive understanding of each patient’s emotional, social, and environmental context relevant to their care.

• Provide empathetic counseling and support to both patients and their families, effectively addressing emotional challenges and assisting with adjustment issues common in serious illness scenarios.

• Develop individualized treatment plans in collaboration with an interdisciplinary team, ensuring psychosocial factors are central to the patient’s overall care strategy.

• Guide patients and families in navigating community resources, including support groups, financial assistance programs, and vocational rehabilitation services.

• Actively participate in discharge planning and facilitate smooth transitions to home or other care environments, preparing patients and families for the next steps in care.

Your primary client focus will be patients with terminal illnesses and their families. Qualifications\n

Compensation and Benefits\n
