This role is primarily remote, enabling you to work from the comfort of your home while collaborating with a dedicated group of professionals. Our work facility operates as a community health clinic in Memphis, Tennessee, where we prioritize patient-centered services. You will have flexible scheduling, with some evening and weekend hours needed to accommodate our clients' various needs.

Key Responsibilities\n

• Utilize your education and training to conduct independent psychosocial assessments, formulating diagnoses that will inform treatment plans.

• Work collaboratively with patients to develop tailored, patient-centered treatment plans that address their unique mental health needs and goals.

• Employ a variety of evidence-based treatments such as solution-focused therapy, motivational interviewing, and behavioral activation to provide effective interventions.

• Coordinate care with multidisciplinary teams including psychologists, primary care providers, and community services to ensure holistic patient support.

• Monitor and track patient progress through in-person or virtual check-ins, adjusting treatment plans as necessary to meet designated goals.

• Assist with psychotropic medication management by focusing on treatment adherence, monitoring side effects, and assessing treatment effectiveness.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current, valid, and unrestricted license to practice independently as a Licensed Clinical Social Worker (LCSW) within Tennessee.

Compensation and Benefits\n
