Key Responsibilities\n

• Conduct comprehensive assessments of patients to determine their mental health needs and tailor individualized treatment plans accordingly.

• Maintain open and effective communication with supervisors, direct care staff, family members, and guardians regarding each patient’s progress and adjustment of care strategies.

• Develop and provide a tentative schedule of appointments, including home visits, to monitor and evaluate patient advancement and barriers to care.

• Coordinate and execute home visits to fulfill the requirements of in-home evaluations, chart reviews, and staff interviews, ensuring thorough data analysis.

• Ensure compliance with the implementation of the program plan, actively monitoring patient adherence and engagement across all managed services.
\n
• Participate in interdisciplinary team meetings and contribute to discussions on treatment strategies, obstacles, and program effectiveness.
\n
• Document and complete monthly and quarterly progress notes that are reflective of patient experiences and outcomes.
\n
• Facilitate training sessions to ensure that all staff members are competent in implementing the program plan and proficient in necessary documentation processes.

• Organize follow-up assessments, home visits, and supportive observations to continuously address the evolving needs of patients.

• Maintain meticulous records to ensure that relevant and necessary information is readily available to patients and team members.

• Act as a liaison with the broader public and mental health community to drive initiatives that support developmental goals and patient engagement.

• Contribute to the creation of procedures and materials that enhance training programs for staff and program improvements.

Qualifications\n
