Noble Healthcare Partners is seeking a compassionate and skilled Licensed Professional Counselor (LPC) to join our dedicated team. In this full-time, remote role, you will be responsible for providing personalized mental health services to our clients through a community health clinic setting, ensuring that they receive the care and support needed to enhance their overall well-being.

Work Environment

This position is entirely remote, allowing flexibility for counselors to manage their schedules while providing essential services. Although working from home, you will engage regularly in virtual meetings and interdisciplinary team discussions to maximize client outcomes and ensure seamless integration of care.

Key Responsibilities

• Conduct thorough and ongoing assessments to accurately evaluate the mental health needs of your clients.
• Maintain open lines of communication with supervisors, care team members, family members, and other stakeholders to discuss individual progress and treatment plans.
• Create and provide tentative schedules for patient appointments, home visits, and therapeutic sessions, ensuring effective management of your caseload.
• Coordinate and conduct home visits as necessary, performing data analysis, chart reviews, and staff interviews to monitor progress and enhance care effectiveness.
• Ensure that the program plan is executed properly and is effective in meeting the needs of each client.
• Actively participate in Interdisciplinary Team Meetings to discuss case reviews, share insights, and keep abreast of best practices in mental health care.
• Complete monthly and quarterly progress notes that reflect client progress and any necessary adjustments to treatment plans.
• Organize and facilitate training sessions for staff on program implementation and documentation to ensure consistency in service delivery.
• Provide follow-up support through interviews and home visits, tailoring interventions to suit individual client needs.
• Maintain accurate and organized records to ensure information is readily accessible for organizational use and client care.
• Act as a liaison between the mental health community and the public, fostering partnerships that support community mental health initiatives.
• Contribute to developing training materials and procedures aimed at enhancing counseling programs and services.

Patient Population / Client Focus

You will have the opportunity to work with a diverse range of clients, including adults and youth facing various mental health challenges. The focus may include areas such as family dynamics, trauma recovery, and substance misuse, making it essential to approach each client uniquely and empathetically, recognizing their individual circumstances and backgrounds.

Qualifications

• A Master's or Doctoral degree in Counseling, Psychology, or a related mental health field.
• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
• At least 2 years of experience in a counseling role, preferably in a clinical setting, to demonstrate competency in client interaction and therapeutic techniques.
• A strong understanding of legal and ethical standards pertaining to mental health practice.
• Excellent interpersonal and communication skills, capable of establishing rapport with diverse clientele.
• Proven ability to work collaboratively within a healthcare team.
• Familiarity with Electronic Medical Records (EMR) software and other relevant technologies.

Preferred Experience

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance abuse counseling will be considered an asset.
• Experience in community health settings or with vulnerable populations is highly desirable.

Compensation and Benefits

• Annual salary range of $90,000 to $100,000, with caseload determined by experience and availability.
