Key Responsibilities\n

• Conduct thorough clinical evaluations of patients upon admission, ensuring their needs are effectively documented and addressed.

• Develop and implement treatment plans that include evidence-based interventions and strategies tailored to each patient's unique conditions.

• Lead therapy sessions and psychoeducational groups, providing patients with the tools they need to cope with their situations.

• Complete discharge sessions to ensure patients understand their coping skills and have a solid transition plan in place before leaving care.

• Maintain ongoing communication with referral sources and outpatient providers to facilitate continuity and coordination of care during the patient’s stay.

• Assist in crisis management and provide immediate support to patients in distress during sessions or throughout their care experience.

• Work closely with the treatment team and milieu staff, promoting effective communication and collaboration for optimal patient care and outcomes.

• Participate in clinical outreach and program tours as part of our commitment to community engagement and support.

• Document services accurately and in a timely manner, including initial assessments, SMART treatment plans, and clinical case notes.

• Prepare and implement discharge planning from the day of admission, ensuring that all resources are identified and provided.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline is required.
