Key Responsibilities

• Conduct comprehensive clinical assessments for patients referred to hospice care, utilizing established best practices and evidence-based interventions to develop individualized treatment plans.
• Implement therapeutic modalities tailored to the emotional and psychological needs of patients, including but not limited to cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT).
• Maintain accurate clinical records and documentation in compliance with HIPAA regulations and organizational policies, ensuring timely audits and quality assurance.
• Collaborate closely with a diverse team of healthcare providers, sharing insights and strategies to ensure comprehensive care and adaptability of treatment plans.
• Participate in regular case consultations, peer review meetings, and continued education opportunities to enhance your professional expertise and service delivery.
• Educate patients and their families on mental health issues, available treatment options, and community resources that promote well-being and coping strategies.
• Stay informed of the latest research trends within the mental health field, incorporating new knowledge to improve patient outcomes and support service innovation.
• Manage a caseload of patients effectively, ensuring therapy goals are met within expected timeframes while working within ethical and professional guidelines set by the National Association of Social Workers (NASW).

Patient Population / Client Focus

In this role, you will work primarily with patients in a hospice setting, who are often facing a range of complex emotional and psychological challenges. You will be pivotal in providing support not only to these patients but also their families, helping them navigate grief, loss, and the emotional burdens associated with end-of-life care.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Must possess a valid license to practice as a Licensed Clinical Social Worker (LCSW) in Texas.
• At least three years of clinical experience in social work, specifically in providing psychotherapy services.
• Strong knowledge of solution-based interventions and their application in clinical practice.
• Excellent communication and interpersonal skills
• Proven problem-solving and critical thinking abilities.
• Ability to collaborate effectively with a multi-disciplinary team and provide patient-centered care.

Preferred Experience

• Prior experience working in hospice or palliative care settings is highly desirable.
• Additional certifications in specific therapeutic modalities (e.g., CBT, DBT) are a plus.
• Experience in community outreach or patient education initiatives.

Compensation and Benefits

The salary for this position ranges between USD 80,000 and 100,000 annually, commensurate with experience.
