Work Environment

As a Home Care Physical Therapist, you will operate primarily in the patients' home environments, thus providing a unique opportunity to engage with patients in the comfort of their own surroundings. Key Responsibilities

• Evaluate patients in their home environments, assessing their mobility, pain levels, and overall functional abilities to develop comprehensive treatment strategies.
• Design individualized treatment plans employing various therapeutic techniques, such as exercise regimens, manual therapy, and advanced modalities to alleviate pain and enhance mobility.
• Utilize evidence-based practice to track patient progress, document treatments administered, and modify intervention plans based on ongoing evaluations and patient feedback.
• Educate patients, caregivers, and family members on appropriate therapy techniques and the safe use of assistive devices to foster independence in daily activities.
• Participate in regular team meetings to collaborate on best practices and share insights on patient progress and challenges within the home care context.
• Stay updated with developments in the field of physical therapy by engaging in continuing education opportunities and attending relevant workshops.

Qualifications

• A Bachelor's Degree in Physical Therapy (DPT required).

Compensation and Benefits
