Key Responsibilities\n

• Develop, coordinate, and manage tailored support packages to assist clients in navigating and overcoming their challenges.

• Conduct thorough interviews and assessments to determine service users’ current conditions, needs, strengths, and weaknesses.

• Set measurable, achievable goals for each client, creating a personalized plan that addresses their specific situations.

• Regularly monitor and evaluate clients’ progress, making necessary adjustments to treatment plans based on their evolving needs.

• Provide informative counseling during sessions, sharing insights and recommendations on the best courses of action.

• Maintain comprehensive and accurate client records, documenting sessions and reporting on clients’ statuses effectively.

• Participate in case conferences and, when necessary, provide testimony in court to advocate for client needs.

• Act as a key-worker, facilitating cooperative efforts with multidisciplinary teams to ensure a holistic approach to care.

• Refer clients to appropriate community services to enhance their recovery process and improve outcomes.

Patient Population / Client Focus\n

You will work with a diverse clientele, including adults and families affected by mental health conditions and substance use disorders. Qualifications\n
