Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, utilizing tools and methodologies aligned with best practices in social work.

• Develop tailored care plans in collaboration with clients and their families, addressing emotional, mental, and social goals.

• Provide ongoing counseling services to seniors, offering emotional support through major life transitions such as relocation to assisted living, bereavement, or chronic illness management.

• Advocate for clients' needs, facilitating access to critical community resources, including financial assistance, healthcare services, and social support networks.

• Collaborate with a multidisciplinary team, including healthcare providers, nurses, and caregivers, to create cohesive strategies for client care.

• Ensure thorough documentation of assessments, care plans, and progress notes using advanced electronic health record (EHR) systems.

• Monitor client well-being through regular check-ins, adjusting care plans as necessary to respond to changing circumstances.

• Educate families on available resources, support groups, and workshops aimed at improving social engagement among seniors.

• Resolve conflicts and facilitate communication among families to maintain a supportive care environment for clients.

You will primarily work with seniors, addressing their unique psychosocial challenges while providing them with tools to navigate their later years with dignity and support. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Valid Colorado state licensure as a Master Social Worker (LMSW) is mandatory.

• At least two years of professional experience in social work, particularly working with the geriatric population or within a community health environment.

• In-depth knowledge of social work principles, aging issues, and relevant community resources.

Compensation and Benefits\n

The salary range for this position is between $60,000 and $100,000 annually, commensurate with experience and qualifications.
