Bright Star Counseling is actively seeking a Licensed Clinical Social Worker to join our dedicated team in providing comprehensive care for seniors in Fort Collins, Colorado. In this full-time, remote position, the Social Worker will play a critical role in assessing the psychosocial needs of seniors, developing individualized care plans, and supporting clients and their families in navigating the complexities of aging.

Regular virtual meetings and communication will keep you engaged with your colleagues and up to date on client progress.

Key Responsibilities\n

• Conduct detailed assessments of seniors’ psychosocial needs, integrating emotional, mental, and situational factors that affect their quality of life.

• Collaborate with clients and their families to create personalized care plans that focus on their specific goals and preferences, ensuring they have a voice in their care process.

• Provide counseling and emotional support to seniors, guiding them through life transitions and challenging circumstances, such as loss or health changes.

• Actively advocate for clients' rights, working diligently to connect them with community resources, benefits, and services essential for their support.

• Engage in regular visits and check-ins to monitor clients’ well-being, assess their living conditions, and address any potential safety concerns.

• Facilitate open communication among family members to resolve conflicts and support the client's best interests in their care.

• Educate clients and families about available programs, support groups, and community resources designed to improve social engagement and overall wellness.

• Maintain accurate and comprehensive documentation of client interactions, progress notes, assessments, and care plans in accordance with ethical standards and regulatory requirements.

Patient Population / Client Focus\n

The primary focus of this role will be on the senior population, including individuals who may be experiencing chronic health issues, mental health challenges, or are in need of assistance with daily activities. The Social Worker will provide critical support for clients who require holistic care tailored to their physical and emotional needs.

Qualifications\n

• Master's Degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience in social work, particularly in a community center or geriatric setting, is required.

Compensation and Benefits\n
