This role offers the flexibility of a remote work setting, with daily responsibilities primarily conducted online. You will collaborate with a team of healthcare professionals, including nurses and caregivers, ensuring comprehensive support for clients in our detox center. Key Responsibilities\n

• Conduct in-depth psychosocial assessments of clients entering detox, identifying their emotional, mental, and social needs.

• Collaborate closely with clients and their families to establish personalized care plans that address their unique treatment goals.

• Provide essential counseling and emotional support to clients and their families, guiding them through life transitions and the detoxification journey.
\n
• Advocate for clients to ensure they have access to necessary community resources and support systems, such as outpatient services and support groups.
\n
• Work effectively within interdisciplinary care teams to coordinate services and enhance the overall quality of care provided to detox clients.
\n
• Conduct regular follow-up sessions to monitor client progress, adjust care plans as needed, and address any safety or wellbeing concerns.

• Facilitate conflict resolution among family members to optimize the support system for clients during their recovery process.
\n
• Educate clients and their families on available resources, resources to alleviate anxiety and promote active engagement in recovery.
\n
• Maintain meticulous documentation of client assessments, treatment plans, and all relevant interactions through our electronic health record (EHR) system.

• Stay informed on best practices and compliance standards in social work, updating skills to adhere to ethical guidelines.

Patient Population / Client Focus\n

This role primarily focuses on seniors and individuals traversing the complexities of detoxification. Our clients often deal with anxiety, depression, and the psychosocial repercussions of substance use. As such, your role will require an understanding of the sensitivities involved in helping clients reclaim their mental health and stability.

Qualifications\n

• Bachelor's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Clinical Social Worker (LCSW).

• A minimum of two years of direct experience in a social work setting, preferably within detox centers or community health.

• In-depth knowledge of social work principles, particularly those addressing addiction and aging-related issues.

• Superior assessment, counseling, and problem-solving capabilities.

Compensation and Benefits\n
