Key Responsibilities\n

As a Licensed Clinical Social Worker at Memorial Care, your primary responsibilities will include:

• Independently managing a designated caseload of patients, assessing social, emotional, financial, and cultural factors to inform comprehensive treatment plans.

• Implementing a variety of therapeutic interventions and modalities to dismantle psychosocial barriers hindering patient transitions through the continuum of care.

• Advocating for patients, facilitating control over their treatment options, and empowering them to express their preferences and decisions.
\n
• Coordinating with the healthcare team to address protective service issues, advanced directives, and critical life care decisions.
\n
• Providing education, resources, and referrals to patients and families, while ensuring that all psychosocial needs are integrated into treatment and discharge plans.

• Delivering crisis intervention services and actively managing treatment plans for high-risk patients encountering complex psychosocial issues.
\n
• Supervising and mentoring social work interns and staff, including performing weekly supervision to demonstrate best practices and promote effective service delivery.

• Participating in specialty team rounds and patient care conferences to optimize collaborative care efforts.
\n
• Staying timely and responsive to psychosocial service requests, ensuring no delays in treatment implementation or discharge processes.

• Maintaining a presence on-call as needed, supporting the medical team in crisis situations within the hospice setting.

Patient Population / Client Focus\n

Your work will primarily focus on hospice patients, addressing their unique needs as they navigate the end-of-life journey. You will be instrumental in offering psycho-social support not just to patients, but also to their families during an emotionally taxing period. Your expertise will help ensure that all care provided respects the wishes of patients and enhances their quality of life.

Qualifications\n

Candidates must possess a Bachelor’s degree in Social Work and have a minimum of 2 years of relevant experience. A valid California LCSW license is required to practice independently and to provide supervision for social work staff.

Preferred Experience\n

We prefer candidates who have experience working in hospice, palliative care, or similar healthcare settings. Familiarity with telehealth practices and strong communication skills will be advantageous in this role as our team seeks to maximize patient engagement and support.

Compensation and Benefits\n

This position offers a competitive salary range of USD 120,000 – 130,000 annually.
