Key Responsibilities\n

• Conduct comprehensive initial assessments and evaluations to determine clients' mental, emotional, and behavioral health status.

• Develop personalized treatment plans that address individual, couple, family, or group needs, continually reassessing their effectiveness over time.

• Facilitate engaging and therapeutic individual and group therapy sessions tailored to the specific challenges of clients.

• Collaborate closely with a multidisciplinary team of healthcare professionals, including psychiatrists, nurses, and social workers, to ensure holistic care for clients.

• Maintain detailed and accurate documentation of client interactions, treatment plans, and progress notes within electronic health record systems such as AVATAR.

• Identify barriers to recovery and implement strategies to overcome these obstacles, promoting client resilience and independence.

• Provide ongoing support to family members and primary caregivers, educating them on managing behavioral challenges effectively.

• Proactively follow up with clients and coordinate care with other professionals to ensure continuity of services.

Patient Population / Client Focus\n

This role primarily serves individuals and families experiencing mental health challenges related to anxiety, depression, trauma, and substance abuse issues. Our diverse client population ranges from children to adults, each presenting a unique set of needs and circumstances. Qualifications\n

• Master's degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LCSW, LPCC, or LMFT in the state of California.

• Success in passing the National Counselor Examination (NCE) and/or the National Clinical Mental Health Counselor Examination (NCMHCE).
