Key Responsibilities\n

• Conduct thorough assessments of the patient's psychosocial needs utilizing evidence-based methodologies.

• Develop and implement personalized treatment plans that address short-term counseling, crisis intervention, and case management.

• Guide patients in navigating entitlement processes including subsidized insurance and social services, ensuring they have access to vital resources.

• Assist patients in obtaining prescription cost assistance through community resources and pharmaceutical programs, thereby reducing healthcare costs and improving medication compliance.

• Act as a patient advocate, effectively collaborating with community agencies to connect clients with additional support services.

• Maintain accurate and timely records of patient interactions and treatment progress, utilizing electronic health record (EHR) systems to facilitate effective communication and continuity of care.

• Coordinate transportation assistance when necessary, ensuring that logistic barriers do not hinder patients' access to necessary services.

• Participate in team meetings and clinical discussions that contribute to ongoing program development and patient progress reviews.

Qualifications\n

• Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling from an accredited institution.

• Possession of an active or independent license: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

Compensation and Benefits\n
