In this remote role, you will be part of an interdisciplinary team of rehabilitation experts, including physical therapists, occupational therapists, and speech-language pathologists, who work together to create personalized treatment plans tailored to meet the unique needs of each student. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, observation, and analysis of medical records to identify emotional, social, and environmental factors impacting students’ rehabilitation and recovery.

• Provide individual and group counseling to students and their families, assisting them in navigating adjustment challenges, emotional health concerns, and practical stressors.

• Work collaboratively with the rehabilitation team to develop and implement tailored treatment plans that address students’ psychosocial needs and facilitate their reintegration into the school environment.

• Assist students and families in connecting with community resources such as support groups, vocational rehabilitation programs, and housing assistance to enhance their post-rehabilitation success.

• Engage actively in discharge planning processes, ensuring smooth transitions for students back to home or community settings, and monitor their continued progress.

• Participate in regular team meetings to discuss case progress and share insights, helping to refine our approaches and meet our organizational goals effectively.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
