Key Responsibilities\n

• Conduct comprehensive assessments to identify patient needs and tailor individualized treatment plans that align with our holistic approach to mental healthcare.

• Maintain open lines of communication with supervisors, support staff, and family members to track patient progress, address concerns, and adjust plans as necessary.

• Develop and provide a tentative schedule of appointments, home visits, and follow-ups, ensuring timely intervention and support for clients as they navigate their treatment journey.

• Coordinate and execute in-home visits and observations, undertaking thorough chart reviews and staff interviews to gather insights that inform care strategies.

• Ensure effective implementation of individualized program plans, adapting strategies based on client feedback and progress monitoring.

• Actively participate in interdisciplinary team meetings, contributing insights and strategies that drive collaborative patient care.

• Document client progress through monthly and quarterly notes, ensuring compliance with both legal and ethical standards.

• Organize and facilitate training for staff to ensure competent implementation of program plans and documentation processes.

• Provide ongoing follow-up interviews and home visits, offering support tailored to the evolving needs of each individual.

• Serve as a liaison with the mental health community to foster partnerships that enhance service delivery and patient outcomes.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Atlantic Health P.C., you will work with a wide range of patients, including both adults and youth, addressing various mental health issues. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.
