Work Environment\n

This position is fully remote, providing flexibility and the ability to work from home while ensuring that client visits and support are met effectively. The typical work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend duties based on client needs. The role necessitates being responsive to urgent situations, so on-call availability might be required as well. Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, employing evaluation tools that consider emotional, cognitive, and social factors.

• Collaborate with clients and their families to create tailored care plans that reflect the client's preferences, strengths, and goals.

• Provide emotional support and counseling, aiding seniors and their families as they navigate difficult transitions, such as chronic illness or end-of-life issues.

• Act as a strong advocate for clients’ rights and accessibility to essential community support services, assuring that they receive all necessary assistance.

• Work within a multidisciplinary team, coordinating with nurses, caregivers, and other healthcare professionals to ensure seamless and effective care.

• Conduct regular follow-up visits via telehealth or in-person to monitor seniors' well-being and living conditions, addressing safety concerns as they arise.

• Facilitate conflict resolution among family members, ensuring effective communication and support throughout the care process.

• Educate clients and families about relevant community resources and support groups, promoting social engagement and enhancing quality of life.

• Maintain meticulous documentation of assessments, care plans, and client progress utilizing electronic health record (EHR) systems.

• Stay up-to-date with current best practices in social work, adhering to ethical standards and regulations that govern the profession.

Qualifications\n

• A Master's degree in Social Work from a CSWE-accredited institution.

• Current and valid licensure as a Licensed Master Social Worker (LMSW) in the state of Florida.

• A minimum of two years of relevant social work experience, with a preference for candidates who have worked in hospice, palliative care, or geriatric settings.

• Strong understanding of social work principles, especially regarding aging populations, mental health, and available community resources.

• Proficient assessment, counseling, and problem-solving skills, with a compassionate approach to care.

Compensation and Benefits\n

This position offers a competitive salary ranging from $60,000 to $100,000 annually, depending on experience and qualifications.
