Key Responsibilities

• Conduct thorough assessments of patients, including evaluating medical histories, performing specialized assessments, and documenting your findings accurately.
• Develop and communicate individualized treatment plans to patients and their families, actively involving them in setting achievable rehabilitation goals.
• Implement evidence-based treatment protocols tailored to each patient's specific needs, constantly adapting as their health evolves.
• Collaborate with a multidisciplinary team, including physicians and support staff, to address changes in patient conditions and modify care plans accordingly.
• Manage discharge planning and follow-up care, ensuring seamless transitions and continuity of therapy.
• Maintain legible and concise documentation of all patient interactions, including initial evaluations, daily encounters, and progress summaries, adhering to best practices in medical reporting.
• Foster a patient-centric environment by promoting excellent customer service and adaptability to enhance access to therapeutic care.
• Actively participate in marketing efforts, both within the hospital and in the community, to raise awareness of our services and initiatives.
• Engage in ongoing professional development and serve as a clinical instructor to enhance the learning experience for students in the field.

Patient Population / Client Focus

Qualifications

• Doctorate in Physical Therapy (DPT) is required.

Preferred Experience

• A minimum of 2 years of experience in a physical therapy setting is preferred but not mandatory.
