You will primarily connect with clients via telehealth platforms, ensuring accessibility and convenience for seniors who may have difficulties attending in-person appointments. Occasional in-field visits may be necessary based on client needs, and a flexible schedule will be required to accommodate such interactions.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' emotional, mental, and social needs, ensuring a holistic approach to their well-being.

• Develop and implement personalized care plans in collaboration with clients and their families, addressing their specific goals, preferences, and challenges.

• Provide counseling and emotional support, helping seniors navigate significant life transitions, such as bereavement, health changes, and social isolation.

• Advocate for clients' rights, connecting them with community resources, services, and benefits they are eligible for, thus enhancing their access to necessary support.

• Engage with interdisciplinary teams, including healthcare providers and caregivers, to coordinate comprehensive care, ensuring that all aspects of the client’s needs are met.

• Monitor and evaluate clients' progress through regular virtual check-ins and adjust care plans as necessary to reflect changing needs or circumstances.

• Educate clients and families about local resources, support groups, and wellness programs, fostering community engagement and support.
\n
• Maintain thorough and accurate documentation of client interactions, assessments, and care plan updates within electronic health record (EHR) systems.

• Stay informed about current best practices in social work, particularly those concerning aging populations, to ensure that care provided is both effective and ethical.

Patient Population / Client Focus\n

This position primarily serves seniors within our community, many of whom experience unique challenges related to aging, including mental health issues, social isolation, and difficulties in accessing essential care services. Building rapport and establishing trust with elderly clients and their families is essential, as is the ability to empathize with their experiences and challenges.

Qualifications\n

• Master's degree in Social Work from an accredited institution, qualifying you for licensure.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.

• Minimum of two years of professional experience in a social work capacity, ideally within a community health setting or focused on geriatric care.

• Comprehensive knowledge of social work principles, aging-related issues, and available local community resources.
\n
• Exceptional assessment, counseling, and problem-solving skills tailored to meet the needs of an older adult population.

• Strong communication and interpersonal capabilities, supporting effective collaboration with diverse teams.

• Proficiency in utilizing electronic health record (EHR) systems and other digital tools for clinical documentation and communication.

Compensation and Benefits\n
