Work Environment

This position is based onsite at an assisted living facility in Riverhead, New York. Here, you will work closely with a diverse patient population that includes elderly individuals facing chronic pain and mobility issues. Our facility is equipped with a range of on-site diagnostic testing tools, therapeutic options, and support services that foster holistic care. This collaborative environment encourages interaction with both clinical and non-clinical staff to create a network of support for our patients.

Key Responsibilities

• Conduct comprehensive patient assessments, including medical history reviews, physical examinations, and diagnostic evaluations to formulate effective treatment plans.
• Develop and implement individualized treatment strategies to address pain management, incorporating evidence-based practices and patient preferences.
• Perform medication management, ensuring appropriate prescribing practices and monitoring for efficacy and safety.
• Educate patients and their families on pain management techniques, lifestyle modifications, and the importance of follow-up care.
• Monitor patient progress through regular follow-ups and adjust treatment plans as necessary to optimize outcomes.
• Maintain accurate and thorough documentation of patient encounters, treatment plans, and progress notes in compliance with healthcare regulations.

Patient Population / Client Focus

In this role, you will primarily focus on elderly patients residing in an assisted living facility. Many of these individuals experience complex pain conditions resulting from various underlying health issues, including arthritis, neuropathic pain, and post-operative discomfort. Your expertise in managing their unique needs will significantly impact their quality of life, fostering independence and functionality.

Qualifications

• Master's Degree in Nursing with a certification as a Nurse Practitioner, with a focus on pain management.
• Current NY State Nurse Practitioner license is required.
• Certification as a Physician Assistant is beneficial.

Preferred Experience

• A minimum of 2 years of experience in a pain management setting, with a proven track record of successfully treating patients with chronic pain.
• Experience working in an assisted living or geriatric healthcare environment is highly desirable.

Compensation and Benefits
