Key Responsibilities

• Conduct thorough assessments to diagnose and treat a variety of acute healthcare issues, including infections, illnesses, and injuries, ensuring that all interventions are evidence-based and patient-centered.
• Prescribe medications judiciously, considering efficacy, safety profiles, and cost-effectiveness to optimize patient care.
• Implement interventions aimed at modifying patient behaviors that contribute to health risks, focusing on preventive care and long-term wellness.
• Educate patients about the self-management of both acute and chronic conditions, tailoring programs and guidance to meet individual needs and circumstances.
• Actively participate in evaluating employee health conditions, offering support and interventions as necessary.
• Utilize the EHR system to document clinical encounters comprehensively, ensuring compliance with policy regulations while enhancing patient care continuity.
• Order, perform, and interpret diagnostic tests, including blood counts, EKGs, and radiographs, to inform treatment plans.
• Develop and adjust treatment plans based on scientific rationale, established standards of care, and evolving best practices.

Patient Population / Client Focus

Your role will involve providing healthcare services to a diverse patient population ranging from children to elderly individuals. You will address the complex needs of patients with varying degrees of health literacy, working to empower them through education and personalized care plans. Your goal is to improve overall patient outcomes through tailored interventions and comprehensive follow-up care.

Qualifications

• A Bachelor’s Degree in Nursing, with a valid New York State license and current registration as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
• A valid and current Nurse Practitioner certification from NYSED.
• Certification in Basic Life Support (BLS) through the American Heart Association (AHA), which can be obtained via facility orientation.

Preferred Experience

While not mandatory, previous experience in a hospital setting as a Nurse Practitioner is highly desirable. Familiarity with a range of medical conditions and the ability to work collaboratively in a multidisciplinary team will enhance your candidacy for this role.

Compensation and Benefits
