Key Responsibilities

• Deliver comprehensive, quality care consistent with the latest clinical guidelines and standards, ensuring optimal patient outcomes.
• Conduct thorough assessments of patients’ health status, taking into account their age-specific needs, and develop individualized treatment plans.
• Collaborate with office staff to implement efficient office practices, enhancing patient flow and care delivery within the facility.
• Serve as a liaison between practice management and hospital administration, communicating updates, addressing concerns, and advocating for patients’ best interests.
• Document patient care accurately, ensure timely communication with patients and their families, and maintain compliance with all regulatory requirements.
• Facilitate quality improvement initiatives aimed at meeting the health system's strategic goals and enhancing patient care processes.
• Actively engage in ongoing education and professional development to remain current with best practices in outpatient clinical medicine.

Patient Population / Client Focus

In this role, the Nurse Practitioner will primarily care for elderly residents within the Assisted Living Facility, addressing a wide range of medical needs from chronic disease management to preventive care. Understanding the complexities associated with geriatric health, you will ensure that care is tailored to each individual's unique situation, providing a holistic approach that encompasses their physical, emotional, and social well-being.

Qualifications

• NYS certification/registration as a Family, Adult, or Pediatric Nurse Practitioner is required.
• AANP Board Certification is mandatory.
• Current DEA license is required.
• Proficiency with ICD10 coding is preferred.
• Ability to secure appropriate privileges at F. F. Thompson Health.

Preferred Experience

• Previous experience as a Nurse Practitioner is preferred, particularly in an outpatient or assisted living setting.

Compensation and Benefits

The position offers a competitive salary range of USD 150,000 - 160,000 per year , commensurate with experience and qualifications.
