Position Overview

Health Partners Wellness Solution, LLC is seeking a dedicated Full-Time Nurse Practitioner (NP) to join our exceptional team at our Assisted Living Facility located in Riverhead, New York. In this role, you will leverage your clinical expertise to provide comprehensive care to our residents, ensuring they receive the highest level of medical attention while navigating the unique challenges of assisted living.

Work Environment

The Nurse Practitioner will work onsite within our Assisted Living Facility, engaging directly with residents, families, and the interdisciplinary healthcare team. Our facility is designed to provide a supportive and caring atmosphere, encouraging meaningful interactions that enhance residents' quality of life. You will be expected to maintain frequent face-to-face contact with residents, ensuring timely and effective delivery of care.

Key Responsibilities

• Maintain accurate and compliant documentation using our electronic medical record (EMR) system to ensure seamless continuity of care.
• Collaborate with local and regional operations and clinical leadership to align services with organizational goals.
• Engage in strategic initiatives aimed at promoting value-based care and improving overall patient outcomes.
• Deliver an exceptional customer experience, fostering trust and high satisfaction among residents and their families.
• Adhere to ethical practices in accordance with the Scope and Standards of Practice, while maintaining board certification.
• Follow all applicable state and federal regulations to uphold the highest standards of care.
• Work in conjunction with Primary Care Providers (PCPs) and the interdisciplinary care team to ensure comprehensive and cohesive treatment plans.
• Coordinate care with external providers, including specialists and telemedicine resources, to optimize health outcomes.
• Conduct all regulatory-required visits, such as monthly, quarterly, or annual assessments, alongside acute visits as necessary.
• Participate actively in family conferences and engage in advanced care planning and education, providing guidance and support to families.
• Address recommendations from pharmacies and effectively communicate with pharmacists regarding resident care.
• Conduct thorough assessments, diagnose, and manage both chronic and acute health conditions.
• Provide necessary health education to residents and their families to promote informed decision-making and self-management.
• Develop and communicate individualized treatment plans to residents and appropriate caregivers, ensuring understanding and adherence.
• Order and interpret laboratory and diagnostic tests, making adjustments to treatment plans as necessary based on findings.

Patient Population / Client Focus

This role primarily serves elderly residents within our Assisted Living Facility, many of whom manage chronic health conditions and require ongoing medical support. As a Nurse Practitioner, you will play a vital role in addressing the complex health needs of seniors, focusing on improving their quality of life through proactive healthcare management. Your passion for serving senior and low-income populations will be invaluable in providing empathetic, individualized care.

Qualifications

• Active DEA license and unrestricted state license in New York.
• Authorization to work in the United States.
• Current certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner certificate program.
• Excellent communication skills with a collaborative mindset, conducive to teamwork in a healthcare setting.
• Demonstrated ability to work flexibly within a dynamic healthcare environment.

Preferred Experience

• Previous experience working with elderly patients or within assisted living environments is highly valued.
• Knowledge of value-based care models and best practices in geriatric care.
• Experience with EMR systems to streamline documentation and patient care processes.

Compensation and Benefits

The Nurse Practitioner position offers a competitive annual salary ranging from USD 150,000 to 160,000.
