Work Environment

This role is primarily onsite, allowing for direct interaction with patients and their families within their homes. The Nurse Practitioner will spend their work hours engaged with patients, ensuring they receive face-to-face interactions crucial for optimal health outcomes.

Key Responsibilities

• Maintain accurate and compliant documentation using our electronic medical record (EMR) system to ensure continuity of care.
• Collaborate with clinical leadership and local operations teams to align with organizational goals and improve patient outcomes.
• Support strategic initiatives focused on value-based care delivery to enhance service efficiency and quality.
• Deliver superior customer service aimed at maximizing patient satisfaction and retention.
• Practice ethically, adhering to professional standards and board certification guidelines.
• Read and comply with all applicable state and federal regulations affecting practice.
• Engage actively with primary care physicians (PCPs) and interdisciplinary teams at facilities to coordinate comprehensive care.
• Facilitate care coordination with specialists, including those utilizing telemedicine, to ensure holistic patient management.
• Conduct regular regulatory visits, including acute evaluations as necessitated by patient needs.
• Participate in family conferences, providing education and assistance with advanced care planning.
• Address recommendations from pharmacists regarding patient medications and treatment plans.
• Conduct thorough assessments, diagnose, and manage both chronic and acute health conditions.
• Provide health education to patients and their families to foster informed healthcare choices.
• Design and articulate detailed treatment plans, ensuring comprehensibility for patients and caregivers.
• Order and interpret lab tests and diagnostics, adapting treatment plans based on findings.

Patient Population / Client Focus

The primary focus of this role is the elderly population receiving home care. Many patients may have complex medical needs due to chronic illnesses, requiring thorough assessments and tailored treatment approaches. Qualifications

• Bachelor’s Degree in Nursing or a related field.
• Graduation from a Master of Science in Nursing program, Doctor of Nursing Practice, or a recognized NP Certification Program.
• Must possess an active DEA license and unrestricted state license to practice in New York.
• National certification through AANP or ANCC.
• Legal authorization to work in the United States.
