Work Environment

This position is primarily onsite in a home care setting, allowing for direct interaction with patients and families in their own environments. The work schedule is designed to accommodate both the needs of our patients and our team's outreach efforts, with operating hours typically from 9:00 a.m. to 6:00 p.m., and some evening clinics extending until 8:00 p.m. Weekend community outreach events or health fairs may also be part of your responsibilities. You can expect an average workweek totaling approximately 40 hours, providing a balanced work-life dynamic while serving the community.

Key Responsibilities

• Conduct thorough health assessments and screenings to identify patients' medical needs and risk factors.
• Diagnose and manage a variety of acute and chronic health conditions, offering tailored preventive strategies and coordinating health promotion activities.
• Develop and implement educational programs in partnership with community organizations, aimed at increasing public awareness and improving health outcomes.
• Provide culturally competent care that respects the backgrounds and beliefs of our diverse patient population, advocating for their rights and accessibility to necessary healthcare services.
• Ensure continuity of care through effective follow-up procedures, including referrals to specialists when necessary.
• Maintain accurate and detailed medical records in accordance with regulatory standards, ensuring compliance and facilitating quality assessments.
• Engage actively in community events, providing vital health screenings and health education to promote wellness.
• Continuously update your knowledge of community health trends, as well as evolving evidence-based practices.

Patient Population / Client Focus

The Family Nurse Practitioner will work primarily with underserved populations in the Riverhead area, addressing a range of health disparities and challenges faced by these groups. This role focuses on delivering compassionate care to families, individuals experiencing chronic illnesses, and those requiring preventive health services. Qualifications

• Current certification as a Family Nurse Practitioner (FNP).
• Active nursing license in the state of New York.
• A Master's degree in Nursing from an accredited institution.
• Proven clinical experience of at least 3-5 years, particularly in community health or public health environments.

Compensation and Benefits

The compensation for this position is competitive, with a salary range of $180,000 to $200,000 annually, depending on experience and qualifications.
