Work Environment

This position is based onsite within a hospital setting in Riverhead, ensuring that you have access to a comprehensive array of healthcare resources and tools. You will operate primarily on weekdays from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Your weekly time commitment will average around 40 hours, with the potential requirement to participate in community outreach initiatives and health fairs on weekends, reinforcing our commitment to community health.

Key Responsibilities

• Conduct thorough health assessments and screenings for a diverse range of patients and families in the community, using advanced diagnostic skills and evidence-based practices.
• Diagnose and manage both acute and chronic medical conditions, crafting personalized care plans that include preventive measures and health promotion strategies tailored to individual patient needs.
• Collaborate extensively with local health organizations and stakeholders to design, implement, and evaluate effective health education programs aimed at improving community health literacy.
• Advocate zealously for patients' rights and their access to healthcare services while ensuring culturally sensitive care that respects the diverse backgrounds of our patient population.
• Coordinate referrals and follow-up care diligently to ensure seamless care transitions, working closely with other healthcare providers and specialists.
• Maintain meticulous medical records, adhering strictly to all regulatory standards to ensure compliance and high-quality patient care delivery.

Qualifications

Compensation and Benefits
