Work Environment

The position is primarily onsite at our hospital facility in Riverhead, New York, where you will interact closely with a multidisciplinary team of healthcare professionals, including pediatricians, nurses, and community health workers. Our work setting promotes a collaborative and supportive atmosphere that values every team member's contributions. You will typically work from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m., and participation in community outreach events on weekends may be required.

Key Responsibilities

• Conduct thorough health assessments, including physical exams and screenings, for individuals and families in the community.
• Diagnose and treat acute and chronic conditions, ensuring appropriate preventive care measures are in place.
• Implement and evaluate health education programs in collaboration with community organizations to promote wellness and disease prevention.
• Provide culturally competent care, advocate for patients' rights, and identify barriers to access that patients may face.
• Coordinate care through referrals and follow-up services to ensure patients receive comprehensive care.
• Maintain detailed and accurate electronic medical records in compliance with healthcare regulations to track patient progress effectively.
• Participate in community health fairs and outreach initiatives to provide health screenings and education.
• Stay informed about community health trends and best practices to continually enhance the quality of care provided.

Qualifications

• Current certification as a Nurse Practitioner.
• Active licensed to practice as a Nurse Practitioner in New York State.

• A minimum of 3 years of relevant clinical experience, with emphasis on community health or public health settings.
• Strong interpersonal skills with proven ability to communicate effectively with diverse populations.
• Demonstrated commitment to social justice and advocating for health equity.

Preferred Experience

• Experience working in community health initiatives, public health campaigns, or similar roles focused on health education and promotion.

Compensation and Benefits

We offer a highly competitive salary range of USD 180,000–200,000 per year, commensurate with qualifications and experience.
