Key Responsibilities\n

• Engage in thorough communication with physicians and other members of the healthcare team to discuss patient progress, identifying needs, concerns, and any potential issues that may arise throughout the treatment process.

• Ensure comprehensive documentation by maintaining accurate clinical records and progress notes in compliance with department policies and regulations.

• Collaborate with the healthcare team to develop individualized treatment and discharge plans based on detailed psychosocial assessments and evidence-based social work methodologies.

• Provide crisis intervention and emotional support to clients, including the assessment and management of behavioral health challenges.

• Assist clients with discharge planning, ensuring seamless care coordination and access to necessary community resources and services, fostering a smooth transition back into the community.

• Consult with family members or support systems, actively involving them in the treatment and discharge planning process to enhance client support networks.

Compensation and Benefits\n
