Key Responsibilities\n

• Conduct thorough assessments to understand the counseling needs of each client seeking treatment.
\n
• Utilize the DSM (Diagnostic and Statistical Manual) criteria to assess clients for substance abuse diagnoses, ensuring accurate identification of their necessary level of care based on ASAM (American Society of Addiction Medicine) guidelines.

• Prepare comprehensive substance abuse counseling components as part of each client's personalized treatment plan.
\n
• Develop effective discharge and continuum of care plans to support clients’ transitions to appropriate levels of care during recovery.
\n
• Maintain meticulous documentation of all counseling sessions, assessments, education services, referrals, and follow-ups within the client's clinical records to ensure continuity of care.
\n
• Manage a caseload of clients, performing ongoing case management duties and updates as required, including clinical charting responsibilities.
\n
• Facilitate coordination of vital support services for clients and their families, maintaining accurate records of the services provided.
\n
• Engage with various referral sources, such as legal, educational, and employment entities, to create a collaborative support network for clients.
\n
• Work as part of a multi-disciplinary team to conduct crisis interventions for clients to ensure timely and effective resolutions to emergent issues.

Patient Population / Client Focus\n

In your role, you will directly engage with a diverse patient population struggling with addiction, supporting their recovery journeys through individual therapy, group therapy, and family sessions. Your expertise will be pivotal in guiding clients towards long-term stability and self-sustainability, thus contributing to the overall health of the community.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited program.

• A valid LCSW license in California.
\n
• Strong communication skills, demonstrating professionalism and empathy in client interactions.

• Proficiency in maintaining therapeutic boundaries and showcasing cultural competency in a diverse environment.

• Self-motivated with excellent time management and organizational skills, enabling you to effectively manage a caseload.
