Work Environment

This position is located onsite at our state-of-the-art rehabilitation facility in Santa Rosa, where you will work closely with a multidisciplinary team, including psychiatrists, psychologists, and social workers. Our center embodies a nurturing atmosphere that encourages professional growth and teamwork. The hours are Monday to Friday, from 9:00 AM to 5:00 PM, with potential flexibility required based on patient care needs.

Key Responsibilities

• Conduct thorough physical assessments to determine patients' strengths, challenges, and specific rehabilitation needs.
• Develop and implement tailored treatment plans utilizing various physical therapy techniques, such as therapeutic exercise, manual therapy, and modality application.
• Educate patients and their families about rehabilitation programs and empower them to take an active role in their recovery process.
• Monitor patient progress closely, adjusting treatment plans as needed to ensure optimal outcomes and adherence to established goals.
• Collaborate with other healthcare professionals to coordinate integrated care and provide holistic treatment options for patients.
• Participate in case reviews and team meetings to discuss patient outcomes and continuously improve service delivery.
• Maintain comprehensive patient records in compliance with legal, ethical, and organizational standards, utilizing electronic health record (EHR) systems.
• Offer support and advice on injury prevention and wellness strategies to enhance patients' quality of life.
• Stay current with physical therapy advancements and emerging best practices through continuous education and professional development opportunities.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including individuals recovering from surgery, sports injuries, chronic pain, and other physical health conditions. Our approach is focused on understanding each patient's unique challenges and goals, allowing us to provide effective and customized rehabilitation services. Qualifications

• Bachelor's Degree from an accredited institution in Physical Therapy or a related field.
• Current and valid physical therapy license in the state of California.
• Strong understanding of rehabilitation science and evidence-based treatment approaches.
• Demonstrated capability to evaluate and design individualized treatment programs for a broad range of physical conditions.
• Exceptional communication and interpersonal skills, enabling effective relationships with patients and team members.
• Commitment to maintaining ethical and professional standards within a clinical setting.
• Proficiency in using EHR systems and an aptitude for adopting new technologies to enhance patient care.

Preferred Experience

• Experience in a clinical rehabilitation setting, particularly within a behavioral health context.
• Proven track record in successfully implementing a variety of physical therapy modalities.
• Previous exposure to collaborative, multidisciplinary team environments.
• Interest in or specialization in areas such as sports rehabilitation, geriatric therapy, or pain management.

Compensation and Benefits
