Work Environment

This role is based onsite in the home care environment, allowing you to work closely with patients in the comfort of their own surroundings. You will adopt a hands-on approach to physically support and rehabilitate patients, ultimately helping them regain mobility and improve functional outcomes. Standard working hours are Monday to Friday, 9 AM to 5 PM, providing a structured schedule to support work-life balance.

Key Responsibilities

• Conduct thorough physical therapy evaluations to assess patients' conditions and develop tailored treatment plans based on their unique needs and goals.
• Implement hands-on therapeutic interventions aimed at enhancing physical capabilities and improving functional outcomes, utilizing evidence-based practices.
• Regularly monitor and document patient progress, making necessary adjustments to care plans to ensure the best possible rehabilitation results.
• Provide education to patients on wellness, injury prevention strategies, and design effective home exercise programs to support their recovery between sessions.
• Collaborate with referring physicians, physical therapist assistants, and other healthcare professionals to coordinate interdisciplinary care that optimally benefits each patient.
• Maintain accurate, timely documentation of patient interactions and treatment sessions within our electronic medical records (EMR) system, ensuring compliance with health regulations.

Patient Population / Client Focus

Your focus will be primarily on orthopedic patients who require rehabilitation following surgery or injury, as well as those with chronic conditions that impact their mobility. You will work with a diverse patient demographic, including older adults seeking recovery from joint replacements, athletes looking to return to peak performance, and individuals needing support for sports-related injuries.

Qualifications

• An active California Physical Therapist License is required.
• A Bachelor's degree in Physical Therapy from an accredited program is essential; a Doctorate in Physical Therapy (DPT) or a Master's in Physical Therapy (MPT) is preferred.
• Exceptional clinical reasoning abilities paired with strong communication skills to effectively interact with patients and healthcare providers.

Compensation and Benefits

We offer a competitive salary ranging from $110,000 to $120,000 per year, commensurate with your experience, alongside a robust benefits package.
