Key Responsibilities

• Conduct thorough examinations and evaluations of patients with functional limitations, disabilities, or orthopedic conditions. Develop tailored treatment plans based on comprehensive assessments.
• Implement and monitor individualized treatment protocols, ensuring adherence to best practices and standards set forth by local and state regulations.
• Provide detailed documentation of patient evaluations, treatment plans, progress notes, and discharge summaries to maintain accurate and complete patient records.
• Actively consult with multidisciplinary teams on patient care, recognizing when to modify treatment plans based on evolving patient conditions.
• Engage in discharge planning and coordinate follow-up care to enhance the transition from rehabilitation to home and community.
• Contribute to organizational goals through participation in team meetings and collaborative initiatives aimed at enhancing patient access and satisfaction metrics.

Qualifications

• Current CPR certification is necessary for maintaining patient safety and care standards.
