Senior Physical Therapist - Home Care at Symbria Rehabilitation Center

Position Overview

We are currently seeking a dedicated and experienced Senior Physical Therapist to join our dynamic team at Symbria Rehabilitation Center. Located in Old Bridge, New Jersey, this full-time position offers a unique opportunity to work directly within our clients' homes, providing high-quality rehabilitative care to enhance their independence and quality of life. Work Environment

As a Senior Physical Therapist in our Home Care setting, you will operate onsite at our clients' residences. This role offers the unique advantage of personalized interaction with patients in a familiar environment, which enhances the effectiveness of rehabilitation therapies. You will be part of a collaborative team that includes nurses, occupational therapists, and other healthcare professionals, all working together to ensure cohesive care for our clients.

Key Responsibilities

• Direct physical therapy treatment aimed at preventing disability, relieving pain, improving muscle function, and maximizing performance based on each client's capabilities.
• Assist and oversee Physical Therapy Assistants to ensure proper implementation of individualized care plans.
• Evaluate clients' functional levels, conducting diagnostic tests on muscles, nerves, joints, and physical abilities.
• Assess the home environment to make recommendations that promote greater independence for clients.
• Provide therapeutic services as prescribed by physicians, including manual exercises and non-manual therapies to motivate and help clients in their rehabilitation journey.
• Administer various physical therapy treatments such as traction and massage, and evaluate and fit prosthetic and orthotic equipment.
• Document clinical findings comprehensively, including initial evaluations, care plans, progress notes, and discharge summaries, ensuring they are available for the client's clinical record.
• Engage in interdisciplinary team case conferences to discuss and evaluate comprehensive treatment strategies.
• Consult and instruct clients and their families on therapy regimens and the care of therapeutic devices.
• Timely submission of orders and clinical documentation according to branch policies to ensure clinical efficiency.
• Complete on-site functional assessments and regularly review the care plan for appropriateness and effectiveness.

Qualifications

• DPT (Doctorate in Physical Therapy) from an accredited program.
• A valid physical therapy license in the State of New Jersey.
• Current CPR certification.

Preferred Experience

While a minimum of two years' experience as a physical therapist is required, preference will be given to candidates with experience in home care or acute care settings. A strong foundation in adjusting to unique client environments and effective communication strategies is also highly desired.

Compensation and Benefits

Symbria offers an annual salary ranging from $100,000 to $120,000, commensurate with experience and qualifications.
