Work Environment

This position requires working onsite in a home care facility, where you will interact closely with patients in their living environments. The supportive atmosphere fosters teamwork among healthcare providers and encourages open communication with families and community members. Your typical work schedule will be Monday through Friday, from 8 AM to 5 PM, allowing for a balanced professional and personal life.

Key Responsibilities

• Conduct comprehensive evaluations of patients, assessing their physical abilities and limitations based on detailed history and specific assessments. Utilize these findings to develop personalized treatment plans aimed at meeting individual rehabilitation goals.

• Implement therapeutic exercises and interventions grounded in research. You will lead one-on-one treatments, ensuring they are modified based on continuous assessment of the patient's condition.

• Document all aspects of patient care accurately and in a timely manner, including evaluations, progress notes, and discharge plans. Our evolving healthcare environment demands meticulous record-keeping, providing a clear picture of each patient's course of care.

• Collaborate effectively with multidisciplinary teams by consulting with physicians and other healthcare professionals as needed, particularly when adjustments to patient care plans are required. Recognizing advancements or changes in a patient's condition is crucial for timely and effective rehabilitation.

• Engage patients in their recovery by encouraging feedback and creating a therapeutic environment that emphasizes open communication. This includes providing education about the treatment process and involving patients in their rehabilitation goals.

• Participate in regular staff meetings and professional development opportunities to enhance your skills and contribute to organizational improvements.

Patient Population / Client Focus

As an Orthopedic Physical Therapist at Breakthrough, your primary focus will be on individuals recovering from orthopedic surgeries, injuries, or chronic conditions affecting their mobility and physical well-being. You will work with a diverse patient population, tailoring your approach to meet the unique needs of each individual, helping them transition from pain to progress successfully.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited program is mandatory.

• A valid physical therapy license in the state of New Jersey.

• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

We prefer candidates with at least two years of experience working as a Physical Therapist, particularly in an orthopedic outpatient setting. Familiarity with the latest rehabilitation techniques and patient management strategies will enhance your fit within our team.

Compensation and Benefits
