Revive Physical Therapy is seeking a dedicated and experienced Physical Therapist to join our expanding home care team in Morristown, New Jersey. Work Environment

As a home care Physical Therapist, you will work directly in patient homes, experiencing the dynamic nature of therapy in various environments. This onsite role allows you to establish a personal connection with patients in their familiar surroundings, enhancing treatment effectiveness and patient comfort. Flexible hours during our Monday to Friday schedule from 9 am to 5 pm are available, enabling you to balance work-life commitments.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions, including mobility, strength, and functionality, to develop tailored treatment plans.
• Implement individualized rehabilitation programs that may consist of hands-on therapies, therapeutic exercises, and innovative modalities such as electrical stimulation or therapeutic ultrasound.
• Instruct patients and their families on proper techniques for home exercises, injury prevention, and ergonomics to facilitate long-term wellness.
• Monitor treatment progress meticulously and adjust plans based on patient feedback and clinical outcomes to optimize recovery.
• Collaborate with a multidisciplinary healthcare team, including physicians, occupational therapists, and nursing staff, to provide holistic care and ensure continuity of treatment.
• Maintain thorough and accurate documentation of patient interactions, assessments, and treatment outcomes in compliance with state regulations and best practices.
• Provide patients with education on lifestyle modifications and wellness strategies to empower them in their own recovery journey.

Patient Population / Client Focus

In this role, you will work with a diverse client population, including individuals recovering from surgeries, neurological disorders, sports injuries, and chronic conditions such as arthritis and neurological impairments. Your approach will consider each patient's specific circumstances, goals, and home environment, allowing for customized interventions that foster quicker and more effective rehabilitation outcomes.

Qualifications

• A Bachelor's or Master's degree in Physical Therapy from an accredited educational institution.
• Current licensure to practice as a Physical Therapist in the state of New Jersey.
• Board certification from the American Physical Therapy Association (APTA) is required.
• At least one year of clinical experience treating a variety of physical therapy conditions.
• Expertise in advanced physical therapy techniques and a strong commitment to continual personal and professional growth.
• Exceptional analytical skills with an ability to create adaptable treatment plans based on patient needs and progress.
• Strong interpersonal skills, fostering effective communication with patients, families, and healthcare professionals.

Preferred Experience

• A minimum of five years in a clinical setting is preferred, particularly in home care and with a diverse range of conditions.
• Experience with electronic medical records (EMR) systems to ensure efficient documentation and patient history tracking.
• Familiarity with community healthcare resources and services that can complement patient care plans and support overall wellness.

Compensation and Benefits
