Wellness Works Physical Therapy, PC, is seeking a dedicated and compassionate Physical Therapist to join our team as a full-time clinician. Based in Fairview, New Jersey, you will have the opportunity to work directly with patients in their home environments, providing tailored therapy services designed to enhance their quality of life. Work Environment

This position is fully onsite, allowing you to work with clients in their homes, where you can assess their specific needs and challenges in real-time. Key Responsibilities

• Direct physical therapy treatment to help prevent disabilities, relieve pain, and improve or restore muscle function based on the individualized plan of care.
• Supervise and mentor physical therapy assistants, ensuring they implement treatment plans effectively.
• Conduct comprehensive evaluations of the client's physical function using diagnostic tests to determine their treatment needs.
• Assess the home environment to recommend modifications that enhance client independence.
• Deliver physical therapy services as prescribed by physicians, including manual therapeutic exercises and instructive support for patients during rehabilitation.
• Administer various treatments such as traction and massage, utilizing physical agents to aid in patient recovery.
• Evaluate and fit prosthetic and orthotic devices, recommending adjustments as necessary.
• Participate in developing and reviewing care plans in collaboration with medical professionals.
• Document progress notes, care plans, and discharge summaries to maintain accurate clinical records.
• Engage with clients and their families, providing education on physical therapy devices and overall care management.
• Remain accessible to physical therapy assistants during treatments to ensure high-quality service delivery.
• Complete other duties as assigned that contribute to the objectives and mission of the organization.

Patient Population / Client Focus

In this role, you will work with a diverse patient population in their own homes, focusing on those recovering from surgeries, managing chronic conditions, or requiring rehabilitation post-hospitalization. Our clients range from elderly individuals seeking to improve mobility to those with specific physical rehabilitation needs. Each patient presents unique circumstances that require your expertise in crafting and executing individualized treatment plans.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid and current physical therapy license in the state of New Jersey.
• A minimum of one year of experience in physical therapy, preferably in a home care or acute care setting.
• Current CPR certification.
• A commitment to maintaining clinical excellence and delivering high-quality care.
• Strong interpersonal and communication skills, essential for engaging effectively with clients and their families.
