Pathways Physical Therapy is seeking dedicated Physical Therapists to join our dynamic team at our state-of-the-art rehabilitation center located in Leesburg, Virginia. This full-time, onsite position offers a competitive salary range of USD 100,000 to 120,000 annually, depending on experience, and is a critical role in providing exceptional outpatient care to our community.

Key Responsibilities

• Conduct thorough evaluations of patients utilizing a detailed review of pertinent medical histories, specific assessments, and systematic documentation of findings to facilitate personalized treatment planning.
• Communicate evaluation results and treatment plans effectively to patients and their families, ensuring inclusion in goal-setting processes that reflect their needs and aspirations.
• Implement individualized treatment protocols based on the results of evaluations, patient characteristics, and current healthcare standards to optimize recovery outcomes.
• Consult with multidisciplinary healthcare professionals when necessary, while recognizing and documenting any changes in patients' conditions that may require plan adjustments or additional consultations.
• Manage discharge planning meticulously by providing follow-up care instructions and coordinating ongoing rehabilitation needs, ensuring a seamless transition in care.
• Maintain accurate and timely documentation of patient care activities, including initial assessments, progress notes, and discharge summaries, all while adhering to evolving healthcare regulations.
• Exhibit strong time management skills by meeting productivity targets and maintaining an orderly schedule that reflects timely patient care.
• Engage in marketing initiatives that promote our services within the community to enhance patient access and care opportunities.
• Contribute to department goals by adhering to compliance and accreditation requirements, and actively participate in professional development activities.
• Encourage patient-centric care by demonstrating flexibility and adaptability to address various patient needs effectively.

Patient Population / Client Focus

Our patient population includes individuals recovering from orthopedic surgeries, sports injuries, neurological conditions, and chronic pain disorders. You will work collaboratively with a diverse group of patients, family members, and caregivers, fostering an environment of encouragement and support that promotes healing and wellness.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A valid state licensure as required by Virginia for practicing physical therapy.
• Current certification in Basic Life Support (BLS) for Healthcare Providers through the American Heart Association.

Preferred Experience

• A minimum of 2 years of professional experience as a licensed Physical Therapist is required, emphasizing direct patient care and treatment planning.
• Experience in outpatient rehabilitation settings, specifically geared towards orthopedic and neurological conditions, is strongly preferred.
