Work Environment

This position is set in a welcoming onsite home care facility located in Savannah, Georgia. Our environment fosters collaboration and teamwork among healthcare providers, ensuring a holistic approach to patient care. You will work primarily during standard weekday hours from 9 AM to 5 PM, with occasional weekend and evening shifts as needed, allowing you to maintain a healthy work-life balance while being available for critical patient needs.

Key Responsibilities

As a Women's Health Nurse Practitioner, your responsibilities will include:

• Conducting initial patient histories and performing thorough physical examinations to assess patient health.
• Creating, documenting, and communicating personalized care plans in collaboration with the patient and interdisciplinary team.
• Conducting daily patient rounds, monitoring patient progress, and adjusting care plans as necessary.
• Writing orders for medications, laboratory tests, and diagnostic studies to inform treatment protocols.
• Interpreting lab results and coordinating with physicians and other healthcare providers to ensure optimal patient care.
• Documenting discharge summaries, prescriptions, and referrals accurately after patient discharge.
• Educating patients and their families about post-discharge care, helping them to understand their health conditions and self-care requirements.
• Performing additional duties related to patient care as necessary to meet organizational objectives.

Patient Population / Client Focus

You will primarily serve a diverse clientele of women, ages ranging from adolescents to senior adults, each with distinct healthcare needs. Your focus will be on delivering compassionate, thorough, and culturally sensitive care, ensuring that every patient's individual health concerns are addressed with dignity and respect. Qualifications

The ideal candidate will possess the following qualifications:

• A Master's Degree in Nursing with certification as a Nurse Practitioner is required.
• National Board Certification in your area of specialty is essential.
• A valid license to practice as a Nurse Practitioner in the state of Georgia and as a Registered Professional Nurse.
• A collaborative practice agreement with designated protocols must be established with the applicable state boards.
• Strong ability to communicate effectively with patients and staff, along with excellent organizational skills.

Preferred Experience

While the minimum required experience is one year in a related role, we prefer candidates with additional experience in women’s health, obstetrics, or primary care settings. Familiarity with electronic health record (EHR) systems and proficiency in using the latest medical software applications is highly desirable, as it enhances efficiency and patient care management.
