Work Environment

This position is onsite at our Assisted Living Facility, creating an intimate and supportive environment for healthcare delivery. You'll work alongside a committed multidisciplinary team, including nursing staff, social workers, and therapists, to coordinate care while fostering an atmosphere of respect and collaboration. The working hours are generally from 9:00 a.m. to 6:00 p.m., with occasional evening clinics that may extend until 8:00 p.m. Additionally, participation in community outreach events or health fairs on weekends may be required as part of your responsibilities.

Key Responsibilities

• Conduct in-depth health assessments and screenings for residents, identifying their individual health needs and managing chronic conditions.
• Diagnose and treat both acute and chronic medical conditions, providing preventive care and promoting overall wellness through education and lifestyle modifications.
• Collaborate with local organizations and health practitioners to design and implement effective health education initiatives that address community health concerns.
• Advocate for patients by ensuring they receive culturally sensitive care and have access to essential healthcare services.
• Coordinate referrals to specialists and ensure seamless follow-up care to enhance continuity of treatment and improve patient outcomes.
• Maintain up-to-date and accurate medical records in compliance with healthcare regulations and organizational policies.
• Engage in community outreach activities and health fairs to raise awareness and provide accessible health screenings to local residents.
• Stay informed about public health trends and evidence-based practices to continually enhance service delivery.

Patient Population / Client Focus

Your work will focus primarily on elderly individuals and those with chronic health conditions requiring regular monitoring and support. You will also engage with families, offering guidance and education to help them participate actively in their loved ones' care. The diversity of the patient population means that cultural competence is essential, and your ability to connect with clients from various backgrounds will be valued.

Qualifications

• Current certification as a Family Nurse Practitioner.
• An active nursing license in the state of Georgia.
• A Master’s degree in nursing from an accredited institution.
• A minimum of 3 to 5 years of clinical experience, preferably in community health or geriatric settings.
• Exceptional interpersonal and communication skills to connect with patients, families, and team members effectively.
• A demonstrated commitment to social justice, health equity, and advocating for marginalized populations.

Preferred Experience

While not required, experience working within assisted living facilities, home health settings, or public health programs is advantageous. Familiarity with electronic health record (EHR) systems and proficiency in using telehealth technologies to manage patient care are also beneficial.

Compensation and Benefits

We offer a competitive salary ranging from $180,000 to $200,000 per year, based on experience and qualifications.
